Joint Projects Grant Application

Application Deadline is July 1

Respond to each of the following in detail. Applications must be word processed. Handwritten applications will not be accepted.

  1. Purpose of project: (Clearly describe the purpose of the project.)
  2. Project objectives: (Define specific objectives of the project in terms of measurable outcomes.)
  3. Strategies: (Describe specific activities. services and steps planned to accomplish the objectives. State the project time line.)  
  4. Evaluation: (State how and when the success of the project will be determined.)  
  5. Funding Request and Budget (Identify amount of funding requested and provide a description of how the money will be spent.)  
  6. Benefits to NCAAHPERD Membership and/or the HPERD profession. (List specific ways the project will benefit the Alliance membership and/or profession.)  

Please be sure to include your home and school contact information, including email address and phone number.

Email the completed application in Word or PDF format to  . Applications sent by other means, such as US Mail will be disqualified.

Joint Projects Grants are selected by the NCAAHPERD Joint Projects Coordinator in cooperation and consultation with the American Heart Association, Mid-Atlantic Affiliate Representatives.
